Loose Adult Tooth — Do Not Wiggle It

An adult tooth that moves is not normal. Sudden looseness is a same-day call.

What is happening?

A loose adult tooth moves under tongue or finger pressure. In adults the leading cause is bone loss from gum disease. Trauma, grinding overload, an abscess at the root, and a failed root canal are the other reasons we see.

Common symptoms

  • A tooth that moves when you touch it
  • A bite that suddenly feels different
  • Swelling or bleeding around one tooth
  • Pain when you chew on it

Is it urgent?

Any suddenly loose adult tooth — same day. Gradual loosening with bleeding gums — same-week periodontal assessment. A loose tooth plus facial swelling — same day, and ER if breathing or swallowing is affected.

If an adult tooth just became loose

  1. Do not test it

    Wiggling it to see how loose it is makes it looser. Call the same day.

  2. Soft food, other side

    Chew away from it until we see you.

  3. Gradual movement is different

    A tooth that has been slightly mobile for months still needs a gum assessment, but it is not the same emergency as one that moved today.

How we diagnose it

We measure how much the tooth moves, check the gum pocket, and x-ray the bone. Sudden mobility after a hit is a trauma exam. Slow mobility with deep pockets is a gum-disease exam. The two visits are not the same.

How we treat it

  • Splint or treat the gum

    A tooth loosened by trauma may be splinted. A tooth loosened by bone loss needs periodontal treatment, and it may not be saveable.

  • Remove only when keeping it is worse

    If the bone is gone, removal and a later replacement can be the cleaner plan. We say that after the x-ray, not from the phone.

Sedation for this kind of treatment

If the loose tooth has to come out and the idea of that is the problem, we can plan sedation for the extraction. The exam itself does not require it. Local anesthetic is what numbs an extraction done the day you call.

Can a loose tooth tighten again?

Sometimes, if the cause is trauma or a bite that is too heavy and the bone is still there. Gum disease that has removed the bone is a different problem, and we will tell you which one the x-ray shows.

Is a slightly loose baby tooth the same thing?

No. Baby teeth loosen on purpose. This page is about adult teeth. If you are unsure which it is, call and describe the age and the tooth.
