Dry Socket — Pain That Starts Days After an Extraction

Dry socket is severe pain a few days after a tooth was removed, often after the first days were improving.

What is happening?

After an extraction the socket should fill with a clot. Dry socket is that clot lost, so bone is exposed. Suction from straws, smoking, or spitting, hard rinsing in the first day, smoking's effect on healing, oral contraceptives, and a difficult extraction raise the chance.

  • Severe throbbing that starts two to four days after the extraction
  • Pain that returns after the first days were improving
  • Pain radiating to the ear
  • A socket that looks empty
  • A bad smell or taste

Is it urgent?

Throbbing that starts days after extraction and is worse than the original soreness — same day. Pain into the ear on that side. You can see bare bone or the socket looks empty.

Until a dry-socket dressing is placed

  1. Call today

    A medicated dressing usually settles this quickly. Over-the-counter pain relief, as the label directs, is only the bridge until you are seen.

  2. Leave the socket alone

    No smoking, no straws, no vigorous rinsing. Gentle salt water only.

  3. Do not pack it yourself

    Cotton, tea bags, and leftover gauze are not a dressing. We place the medicated one.

How we diagnose it

The timing is the clue: pain that worsens on days two to five, an empty socket, and pain that does not match a normal bruise. We look in the socket. We do not need you to diagnose it before you call.

How we treat it

  • Medicated dressing

    We gently rinse the socket and place a dressing. Relief is often quick. You may need the dressing changed once.

  • Healing after that

    The socket still closes on its own over days. The dressing treats the pain. It is not a second surgery.
