Bad Breath — Usually the Mouth, Sometimes Not

Persistent bad breath with a clean routine is a reason for an exam, not another mint.

What is happening?

Halitosis is odour others can notice. The usual sources are a coated tongue, gum disease, a dry mouth from medicine or mouth-breathing, garlic and onion and coffee, smoking, untreated decay, or tonsil stones. Reflux and uncontrolled diabetes are occasional causes outside the mouth.

Common symptoms

  • Someone you trust confirms it
  • A white coating on the tongue
  • A bad taste that does not clear
  • A dry mouth through the day
  • People stepping back when you talk

Is it urgent?

It is still there after two weeks of tongue cleaning, flossing, and water — book an assessment. It arrived with bleeding gums or a bad taste from one tooth — book, and mention which tooth. This is a routine exam, not an emergency, unless there is swelling or pain.

Two weeks of home care, then an honest answer

  1. Clean the tongue

Once a day, plus floss, plus more water. Sugar-free gum between meals helps if your mouth is dry.

  2. Give it two weeks

If the odour is still there after that, book. The cause is usually gum disease or dryness.

  3. We will say if it is not the teeth

Occasionally the source is reflux or another medical issue. We tell you when a physician should look, instead of selling a rinse.

Common causes

Bacterial coating on the tongue · Gum disease · Dry mouth from medication, mouth-breathing, or age · Diet — garlic, onion, coffee · Smoking · Untreated decay · Tonsil stones · Occasionally reflux or uncontrolled diabetes

How we diagnose it

We look at the tongue coating, the gums, dryness, and any tooth that smells when it is flossed. We ask about medicines and reflux. We do not hand you a mouthwash as the examination.

How we treat it

  • Clean what is actually coated

Tongue cleaning, gum treatment, or a decaying tooth — whichever the exam finds.

  • Name a non-dental cause

If the mouth is healthy and the odour remains, we say so and point you toward a physician rather than repeating the cleaning.

I brush twice a day and my breath is still bad. Why?

Brushing misses the tongue and the gum line. If two weeks of tongue cleaning and flossing change nothing, book — we look for gum disease, dryness, or a cause outside the mouth.

Will a stronger mouthwash fix it?

It covers odour for a short time. It does not treat a coated tongue or periodontitis.

Could this be a medical problem?

Sometimes. Reflux and uncontrolled diabetes are examples. If the dental exam is clear, we tell you that directly.

These are common patterns, not a diagnosis of your tooth. Your dentist determines the cause during an examination.
